The day of your consultation we ask that you arrive a few minutes early to begin filling out paperwork. When you check in at the reception desk, we will ask you to provide basic information for our administrative records.

We will also ask you to complete a brief health history which gives the doctor the information he needs to be sure that you are a good candidate for elective surgery. While we try to see you in a timely manner, please understand that some days are busier than others, and we are doing our best to not keep you waiting, both in the reception area and the exam room.
